Infections, arthritis and autoimmune care under one roof

A-One Centre for Infectious Diseases, Rheumatology & Specialty Care brings specialist doctors, a vaccination centre, lab collection, day-care infusions and a pharmacy together in Kudlu, Bengaluru.

OpeningWe open on 10 November 2026. Pre-book your first visit now.

Fever clinic

Fever for 3 weeks or more and no answer yet?

Fever of unknown origin is best evaluated by an infectious diseases specialist and a rheumatologist together, because infections and autoimmune diseases can look identical but need opposite treatment. At A-One, both see you under one roof.

Our specialties

Two specialties that work best together

Many autoimmune conditions are treated with biologics, steroids and other immunosuppressive medicines, and many infections cause joint pain, fever or rashes that look like arthritis. Having an infectious diseases specialist and a rheumatologist in the same clinic means faster answers and safer treatment.

Joint care for immunocompromised patients: on biologics, steroids, chemotherapy and other immunosuppressantsScreening for TB and hepatitis before biologics, vaccines planned before treatment starts, and quick help if a fever appears during treatment.

DiagnoseThe right tests at the right time, read by specialists, so treatment is based on evidence rather than guesswork.
TreatTargeted antibiotics and modern arthritis and immune therapies, used carefully, with day-care infusions on site.
ThriveLong-term follow-up so you get back to work, family and movement, and stay well.

    Infectious diseases specialist

When should you see an ID specialist?

    Consult if you have

    • Fever for more than a week, or fever that keeps coming back
    • An infection that isn’t improving after one or two courses of antibiotics
    • The same infection again and again (urine, skin, chest)
    • A culture report showing a “resistant” or “MDR” organism
    • A new diagnosis of TB, HIV, hepatitis B/C or a fungal infection
    • An infection in a diabetic foot, bone, joint, heart valve or implant
    • Fever or infection while on biologics, steroids, chemotherapy or after a transplant
    • A possible exposure to HIV (within 72 hours) or an animal bite
    • Travel plans, or illness after returning from travel

    Why it matters

The right diagnosis, soonerMany fevers and infections look alike. An ID specialist knows which tests to do, and when, so the cause is found rather than guessed.
The right antibiotic, not the strongestChoosing the narrowest effective drug, at the right dose and duration, cures the infection and protects you from side effects.
Fighting antibiotic resistanceUnnecessary or wrong antibiotics breed resistant bacteria. Careful prescribing keeps antibiotics working for you and your family.
Better outcomes in serious infectionsResearch shows that involving an ID specialist improves survival and reduces relapse in serious infections such as Staphylococcus aureus bloodstream infection.
Fewer hospital staysSwitching to tablets at the right time, or giving IV antibiotics in day care, often avoids admission.

    Rheumatology

A rheumatologist treats much more than “arthritis”

Rheumatology is the specialty of the immune system. Autoimmune and inflammatory diseases can affect almost any organ, and they often begin with symptoms that don’t look like joint disease at all. A rheumatologist connects these clues, confirms the diagnosis and controls the disease before it causes lasting damage.

BloodLow platelets, anaemia or repeated miscarriages from antiphospholipid syndrome.

See a rheumatologist if you have

    • Joint pain and swelling for more than 6 weeks
    • Morning stiffness lasting more than 30 minutes
    • Back pain that is worse with rest and better with movement
    • Fingers turning white or blue in the cold
    • Rashes, mouth ulcers or hair loss with tiredness
    • Muscle weakness or unexplained muscle pain
    • Fever with joint pain, or fever without an infection found
    • A positive ANA, RA factor or other autoimmune test you’re unsure about
    • Repeated miscarriages or blood clots at a young age
    • Long-term steroid use and concerns about bone health
Infections and autoimmunity overlapFever in someone with lupus or on biologics can be a flare or an infection. At A-One, the rheumatologist and the infectious diseases specialist can review such cases together.

    Fever clinic · Kudlu, Bengaluru

Fever of unknown origin (FUO / PUO)

Fever that has lasted three weeks or more, with no cause found despite tests, is called fever of unknown origin (FUO), also called pyrexia of unknown origin (PUO). At A-One, an infectious diseases specialist and a rheumatologist evaluate it together.

    Understanding FUO

What counts as fever of unknown origin?

    Doctors use the term when:

    • the temperature has been 38.3 °C (101 °F) or higher on several occasions,
    • the fever has lasted three weeks or longer, and
    • no cause has been found after basic evaluation (several outpatient visits or a few days in hospital).

FUO is not a single disease. It is a puzzle with a long list of possible causes. Most can be found and treated once the right tests are done in the right order.

Go to an emergency department now if
    • Drowsiness, confusion or a seizure
    • Breathlessness or chest pain
    • Very low blood pressure, cold hands and feet, or fainting
    • Bleeding, or a rapidly spreading rash

    Possible causes

    Four groups of causes

InfectionsThe most common group in India: tuberculosis (often outside the lungs), typhoid, scrub typhus, brucellosis, abscesses, heart-valve infection, HIV, and fungal or parasitic infections.
Autoimmune & inflammatory diseasesAdult-onset Still’s disease, lupus, vasculitis (such as Takayasu or giant cell arteritis), polymyalgia rheumatica, sarcoidosis and inflammatory bowel disease.
CancersEspecially lymphoma and other blood cancers, which can present with fever before any other sign.
Other causesDrug fever, thyroid inflammation, blood clots, and periodic fever syndromes. In some people no cause is found and the fever settles on its own.

    Why A-One

Why FUO is best evaluated by an ID specialist and a rheumatologist together

The two biggest treatable groups of FUO, infections and autoimmune or inflammatory diseases, can look exactly alike: fever, joint pain, rash, weight loss and raised inflammatory markers.

This matters because the treatments are opposite. Autoimmune fever usually needs steroids or other immunosuppressive medicines, which can make a hidden infection such as tuberculosis much worse. Treating an autoimmune fever with repeated antibiotics only delays the diagnosis.

When an infectious diseases specialist and a rheumatologist review the same patient, infections are actively ruled out before immunosuppression is started, and autoimmune causes are considered early instead of after months of antibiotics.

One history, two expert viewsClues such as travel, animal contact, joint pain, rashes, eye or mouth symptoms are interpreted by both specialties.
Targeted tests, in the right orderCultures and TB tests before antibiotics or steroids; autoimmune markers, scans and biopsies only when they will change the answer.
Safer treatmentNo “trial” of steroids or antibiotics without a clear reason, and TB and hepatitis screening before immunosuppression.
Fewer repeated testsA single plan avoids duplicate investigations and shuttling between departments.

    The pathway

How FUO evaluation works at A-One

    1. ReviewSend your reports on WhatsApp before the visit so nothing is repeated unnecessarily.
    2. Joint assessmentDetailed history and examination, covering both infectious and autoimmune clues.
    3. Focused testsBlood cultures, TB tests, autoimmune markers and imaging, collected at the clinic where possible.
    4. DiagnosisBiopsies or advanced scans arranged only when needed, with both specialists reviewing results.
    5. Treatment & follow-upTreatment started once the cause is clear, with follow-up until the fever settles.

    Before your visit

    What to bring

    • A fever chart: temperature readings with date and time
    • All blood tests, cultures, scans and biopsy reports
    • Discharge summaries from any hospital stays
    • A list of every medicine taken, especially antibiotics and steroids
    • Recent travel, animal contact, and any TB contact in the family
Should I start antibiotics while waiting?

Avoid starting antibiotics or steroids on your own. They can hide the cause and make cultures and biopsies less useful. Paracetamol for comfort is usually fine.

Will I need to be admitted?

Most FUO evaluations can be done as an outpatient. Admission is advised only if you are unwell or need procedures.

Can I start with an online consultation?

Yes. Send your reports on WhatsApp and we will advise whether to start online or in person.
